DESCRIPTION:This workshop will encourage participants to consider their wishes for their medical care in the future\, in emergency situations\, and for end-of-life care\; and ensure those wishes are legally documented. The Five Wishes is an easy to understand\, legally binding advanced directive. It is simple to fill out\, no lawyer or notary necessary! The Five Wishes also addresses social\, emotional\, and spiritual needs. Each wish will be explained in detail.\n\n\n\nHosted by Sulzer Regional Library and presented by Maureen Burns\, a Community Health Worker with JourneyCare.\n\n\n\nHow to Attend\n\n\n\nThis event takes place on Zoom. Register at least 24 hours before the event. Only one registration per household is needed. You'll receive an email with a link to the secure Zoom meeting about 24 hours before the meeting. By registering for this event\, you agree to abide by the Library Use Guidelines.\n\n\n\nAccessibility\n\n\n\nAutomatic transcription is included in all CPL events using Zoom.
